The Teen Center is at 94 Main, in the basement of the Middlebury Municipal Building. Enter at the Russ Sholes Senior Center ramp on College Street, across from Middlebury Market and Cafe. All teens who come to 94 Main and their parent or guardian need to fill out a registration form. Download and print the Teen registration form
