Volunteer Application Form: Volunteers need to fill out an application (Microsoft Word DOC file) before they can work at 94 Main, and are subject to a background check.
Fingerprinting Form: Please download the Fingerprint Authorization Certificate that you'll need to submit.
Volunteer Handbook: Please also take the time to download the volunteer handbook (PDF, 8 pages).
